EMR Control Room

One Screen for Every Chart, Every Claim, and Every Open Item in Your Agency

The EMRxAI Control Room is a live operations view for home health agencies. Each patient shows a survey-readiness score, exactly what is missing from the chart, and where their claim stands - so administrators fix problems the day they appear instead of discovering them in an audit.

Built specifically for Medicare-certified home health agencies. HIPAA-compliant by design.

Why agencies find problems too late

In most agencies, chart status lives in one report, orders in another, and claims in the billing system. Nobody sees the full picture of a patient until a chart audit, an ADR, or a surveyor pulls it.

By then, an unsigned verbal order is weeks old, a supervisory visit was missed, and the claim is already held or denied.

What the Control Room shows

Survey readiness per patient

A live score built from the items surveyors check under the Conditions of Participation, rolled up by branch, discipline, and clinician.

Open chart items queue

Unsigned orders, overdue aide supervision, missing consents, late OASIS, and pending physician signatures in one prioritized list.

Claim status synced from the chart

Every claim's stage - building, held, submitted, paid, or rejected - shown beside the documentation it depends on.

Chart export for surveys and ADRs

Export a complete, organized chart for a surveyor or additional documentation request in one action.

How agencies use it each day

  1. 1

    Morning check

The administrator or DON reviews readiness and open items across the census.

  2. 2

    Assign and route

Items route to the responsible clinician, scheduler, or physician office.

  3. 3

    Resolve

As documents are signed and visits completed, scores update in real time.

  4. 4

    Release claims

Held claims release automatically once the chart behind them is complete.

Built on the Home Health Conditions of Participation

Readiness items map to the CoPs - patient rights (§484.50), comprehensive assessment (§484.55), care planning and orders (§484.60), aide supervision (§484.80), emergency preparedness (§484.102), and clinical records (§484.110).

What the Control Room does not do

  • It does not guarantee a deficiency-free survey; surveyors apply their own judgment to the record and observations.
  • It cannot sign documents on behalf of clinicians or physicians - it routes them and tracks status.
  • State-specific requirements beyond federal CoPs need to be configured with your agency.

Frequently asked questions

Who is the Control Room for?

Administrators, directors of nursing, clinical managers, QA staff, and billing leads - anyone who needs to know the real state of the agency right now.

How do claims stay in sync with charts?

Claims are built from the signed documentation in the same system. If a required document is missing or unsigned, the claim is held and the reason is shown in the Control Room.

Can I see readiness by clinician?

Yes. Readiness and open items roll up by clinician, discipline, and branch so you can target coaching where it is needed.
