Survey Readiness

Be Survey Ready Every Day - and Correct Deficiencies Without the Scramble

EMRxAI tracks the chart items surveyors check under the Home Health Conditions of Participation on every active patient. When an agency does receive a deficiency, EMRxAI finds every affected chart, helps draft the plan of correction, and turns the finding into a real-time rule so it does not repeat.

Built specifically for Medicare-certified home health agencies. HIPAA-compliant by design.

Why survey prep is a fire drill

Agencies often prepare for surveys with mock audits on a small sample of charts. The issues found are fixed, but the same gaps keep happening in charts nobody pulled.

When a deficiency is cited, the plan of correction is due fast - and proving the fix is working means manual audits for months.

What every patient chart needs

Rights, consent, and assessment timing

Signed consent and notice of rights (§484.50), initial assessment within 48 hours, and comprehensive assessment within 5 calendar days of SOC (§484.55).

Medications and plan of care

Drug regimen review with physician follow-up (§484.55(c)(5)) and a signed plan of care whose frequencies match delivered visits (§484.60).

Orders, F2F, and homebound status

Authenticated verbal orders, a documented face-to-face encounter, and homebound status supported in the visit notes (§424.22).

Supervision, emergency plan, and summaries

Aide supervision every 14 days when skilled services are in place (§484.80(h)), an individual emergency plan (§484.102), and timely transfer and discharge summaries (§484.110).

Correcting a deficiency with EMRxAI

  1. 1

    Find every affected chart

Scan the whole census for the cited issue, not only the surveyor's sample.

  2. 2

    Find the root cause

Break results down by clinician, discipline, branch, and document type.

  3. 3

    Draft the plan of correction

AI drafts corrective actions, systemic changes, monitoring, owner, and dates for your administrator to edit.

  4. 4

    Block and monitor

The finding becomes a real-time QA rule, and audit results feed QAPI as evidence for the revisit.

Plan of correction timing

After receiving a statement of deficiencies (CMS-2567), agencies are generally required to submit an acceptable plan of correction within 10 calendar days. Your agency's administrator remains responsible for the content and implementation of the plan.

What EMRxAI does not replace

  • Surveyors also observe home visits and interview staff and patients; chart readiness is one part of a survey.
  • AI-drafted plans of correction are starting points that must be reviewed and owned by agency leadership.
  • State licensure surveys may include requirements beyond the federal CoPs.

Frequently asked questions

What do home health surveyors look at in a chart?

Commonly: consent and patient rights, assessment timeliness, OASIS accuracy, drug regimen review, a signed plan of care that matches visits, authenticated orders, aide supervision, care coordination, emergency planning, and transfer and discharge summaries.

How long do I have to submit a plan of correction?

Generally 10 calendar days from receipt of the CMS-2567. Confirm the exact deadline in the letter from your state survey agency or accrediting organization.

Does EMRxAI write the plan of correction for me?

It drafts one from your findings and data. Your administrator edits, approves, and submits it.
